First just listening to it, counting how long it takes between every inhale and exhale. Then eventually he matches it to his own. It doesn't matter if it's fast or slow, it just gives him something else to concentrate on for long enough that he eventually relaxes. The pain still not gone, but at least a bit more manageable.
